It was started for my son, but we had a hard time getting the MR from the hospital, they finally sent a letter saying he's not elgibile, but I know he is. One of his teachers were suppose to resend the App, but not sure if he has, who on the SPED Pac Team, can help us get this restarted.

Direct message from Lena: My daughter attends a school out of the district. She recently turned 20, I have full guardianship. The transition plan form has not been incorporated into her IEP to my knowledge, should I request that to be included in her next IEP. I am in the process of completing the DDS paperwork and do not want this to impact the application process. Thank you.

I would recommend that you speak with your son's Coordinator of Special Education at his school. The TPF is a living planning document that informs transition efforts as they advance through school. This document should inform the IEP and supports and services he should receive towards your son's personal vision.
